The role of the primary care physician in the management of chronic diseases is increasingly important today. The burden of Type 2 Diabetes Mellitus (T2DM), Chronic Kidney Disease (CKD), Heart Failure (HF) and Dyslipidemia is rising, with new studies and guidelines coming to light emphasizing the importance of cardiorenal protection in these diseases.
